Wuhan novel coronavirus (WN-CoV) & Avian Flu

As you have heard on the news the WuHan Flu is spreading

COVID-19 (WuHan) – 1+2

1. Symptoms – either:

  • Acute respiratory infection of ANY degree of severity (including at least one of: shortness of breath, cough or sore throat)
  • Severe acute respiratory infection requiring admission to hospital with clinical or radiological evidence of pneumonia or acute respiratory distress syndrome

2. Travel or Contact history

  • Travel to CHINA/THAILAND/JAPAN/Rep of KOREA/HONG KONG/SINGAPORE/MALAISIA/MACAU in the 14 days before onset of illness? – [UPDATE]
  • Contact with a confirmed case of WN-CoV in the 14 days before onset of illness?

 

AVIAN flu definition – 1+2

1. Symptoms :

  • History of fever or fever ≥ 38°C AND (any of)
    • Lower respiratory tract symptoms (cough or SOB)
    • CXR findings of consolidation
    • ARDS
    • other severe illness suggestive of infectious process

2. Contact history

  • Close contact (within 1 metre) with live, dying or dead domestic poultry or wild birds, including live bird markets, in an area of the world affected by avian influenza or with any confirmed infected animal, in the 10 days before the onset of symptoms
  • In the 10 days before the onset of symptoms: Close contact with a confirmed human case, human case(s) of unexplained illness resulting in death from affected areas, human cases of severe unexplained respiratory illness from affected areas)

For Either: Patient to wear face mask until in Negative Pressure Isolation

Isolation

  • Where? CRH-Minors, HRI-GP room
  • Designated staff: Timed record of contact – on SOP
  • PPE: Long sleeved gown, Gloves, Eye protection, FFP3 mask (if fit tested),Positive pressure hood (if shown how by competent user)
    • IN: Gown>FFP3/HOOD>Eye Protection>Gloves
    • OUT (before leaving room): Gloves+Gown(role off as one and dispose)>Wash Hands>Eye Protection (if not part of hood)
    • OUT (after leaving room):
      • Remove FFP3/HOOD – place on trolley outside room
      • Decontaminate Hands – apply clean apron and gloves
      • Decontaminate MASK/HOOD – use TRISTEL (leave filters in place)
      • Decontaminate Trolley – use TRISTEL
  • Destinations:
    • Ward care (Adult) – CRH ward 5’s negative pressure room
    • Ward care (Kid) – CRH Paeds ward 3’s negative pressure room
    • ICU – negative pressure if available (if not side room)

Call Microbiology

Before sending any samples, microbiology oncall should be contacted

  • 1x Sputum sample, 2x duplicate sets nose and throat swabs in viral transport media
  • Treat as BIO-HAZARD
    • Infection risk label
    • Double Bag
    • Inform Lab sample is coming
    • DON’T SEND IN POD 
  • NO POCT Flu sample to be done!!
