Self Discharge
- Explain why we would like them to stay
- Assess mental capacity (over 16 years)
- Complete self discharge paperwork
- Under 18’s, legally don’t have the right to refuse treatment
Return to Custody
- Relevant advice given
- What medication was given in ED
- Discharge medication
- Capacity assessment if leaving against advice
SUDIC
All information held within SUDIC paperwork
- Care for the child with dignity
- Retain ALL clothing/equipment
- Before removing/cutting ANY thing – You must talk to Coroner
Death in ED
Referred to the Coroner immediately (24 hrs/day)
- Complete Coroners Referral form
- Send via NHS email to cah-tr.coronerreferrals@nhs.net

Locum IT Access
Locum Doctor IT access
- Locum doctors to be assigned a unique log in by Flexible Workforce when booked for a shift.
- If the details are not communicated prior to starting the shift, this should be escalated to Flexible Workforce in hours, and the On Call IT team out of hours.
- Log in details can expire if they are not used frequently, in these instances the IT department can reactivate them.
- Locum logins should never be used due to issues with Information Governance.
